Remove References to Previous Host from MySQL Remote Hosts when Transferring Account

When an account is transferred from another host, the Remote MySQL interface contains references to the losing host, including losing host IP address/es and hostname/s.

These should be removed, for security reasons:

Importing them to the gaining server at time of transfer means that the losing host is now considered an approved Remote MySQL host.

Of course, it may also contain references to real approved remote hosts, and these should remain behind, but WHM can detect which hosts are references to the losing server.
